REQUIRED HEALTH INFORMATION

Healthcare Compliance Information: As an incoming student you are required by Florida law to provide health history and immunization information to the Health Compliance Office before you will be able to register for classes. This is a one-time requirement. To access the required forms, click here.

Health Insurance: Florida State requires full-time students to have health insurance. You must either enroll in the University-sponsored plan or provide proof of comparable health insurance coverage by completing a waiver online. You will be unable to register for classes until this annual requirement is met. If you elect to enroll in the University-sponsored plan, charges will be assessed to your student account, and the cost of meeting this requirement will be included in your financial aid considerations. To learn more about the mandatory health insurance requirement and the costs associated with the University-sponsored plan, click here.

Health Insurance Subsidy: Qualified graduate students will be provided a health insurance subsidy toward the purchase of the University-sponsored health insurance plan. For more information about the subsidy and to see if you qualify, click here.

The Online Status Check will indicate if your immunization records are complete and if you are in compliance with the health insurance requirement. If your Online Status Check indicates non-compliance with either immunizations or insurance, you may contact 850.644.3608 for assistance.

CHILDCARE SERVICES

Florida State has two childcare centers: The FSU Children's Center and the Infant and Toddler Child Development Center. RESIDENCY

Important information about residency is available online. If you are currently an in-state resident, you must submit the online Florida Residency Form to the Office of Admissions. You need to do this even if you were previously classified in-state by FSU or any other Florida institution.

For questions regarding residency classification for newly admitted graduate students, email graduateadmissions@admin.fsu.edu or call (850) 644-6200. Please note: International students are not eligible to apply for Florida residency.

Note: Within very limited and specific conditions, state law does allow for students to petition for a change in residency status after residing in the state for twelve months. It is important to note Florida law sets a high standard for reclassification and numerous date-specific documents are required. It is to your benefit to review the information regarding reclassification very carefully.

For questions regarding residency reclassification, please email registrar@admin.fsu.edu or call (850) 644-1050.

INTERNATIONAL STUDENTS

If you are an international student requiring an F-1 or J-1 visa, you must submit the Certification of Financial Responsibility (CFR) to the Center for Global Engagement (CGE). If you have already submitted the required documents, the CGE will send a packet containing your I-20 or DS-2019 and other information pertinent to your enrollment into Florida State; if you have not submitted the appropriate documentation, you should do so as soon as possible. All international students requiring an F-1 or J-1 visa must obtain the visa prior to entering the United States.

If you are coming from another institution in the U.S., you must submit the Transfer In Form. Upon your arrival, you are required to report to the Center for Global Engagement (CGE) to have your visa status validated in the Student and Exchange Visitor Information System (SEVIS) of the U.S. Department of Homeland Security. If you do not report to the CGE in person and attend the mandatory International Student Orientation, you will be unable to register for classes. Please note that international students are required to enroll in a full course of study.

For more information, visit the CGE website, contact them at 850.645.1424, or email CGE-NewStudents@admin.fsu.edu.
